INSTRUCTIONS
FOR FORMS PRINTOUT:

Click on either the Electrical Contractor's area OR the Homeowner's area. The proper
form will appear on your screen. Depending upon
the speed of your computer and the speed of
your modem (and connection) it might take a few minutes for the form to fully appear. Please
be patient. Use the PRINT button on your web browser OR
the PRINT option listed under
the FILE area in the upper left hand
portion of your screen. This will printout the proper form
on your
computer's printer.

Complete
the form and fax it (to the fax number shown on the form) or mail
it to our office.

|
If
you do not have a printer capable of printing this form, simply
call our office, and we'll be
happy to send you the proper form.

Once
the form has finished printing, press the BACK button at the top
of your web browser, to
take you back to the preceding screen.
